Household ID E-mail Form

If you have registered for a Park District program in 2007, we already have your Household ID set up! We're mailing out this information in letters starting July 9, 2007. If you do not receive your Household ID in the mail, it may mean that we have an incorrect address on file for you! Please fill out the form fields below, and we'll check what you submit against the information in our records. If it matches, we'll send you your Household ID number to the e-mail address you provide. If there is a discrepancy, we'll contact you.
